Five Wishes is an simple document to capture end-of-life wishes and jumpstart important conversations with your loved ones. Written in everyday language that’s easy to understand, it can be completed online (or by hand)—either with The Circle or on your own.

Find the free advance directive form for your specific state. When planning future medical care, advance directives help ensure loved ones make health choices according to your wishes. Forms can be completed on your own or with support from The Circle.

StoryCorps
A great legacy project option, StoryCorps gives people the opportunity to record 40-minute conversations and archive them at the Library of Congress. Recordings can be made via your web browser or the StoryCorp App—and on your own or with help from The Circle.

You’ll craft letters to be delivered after your death using prompts, ideas, and even samples that The Circle has created.

A Life File is a central place to keep practical documents so your loved ones have the information they need to manage your affairs after you die—like accounts and passwords, insurance plans, or even your wishes for your social media. You’ll use the checklist, templates, and ideas The Circle has created to compile a file that meets your specific needs.

A Vigil Plan is an opportunity to imagine, explore, and document your vision for your end of life. Use directions, exercises, ideas, and samples from The Circle to create a concrete yet flexible plan that clearly expresses how you want your final days to look, feel, smell, or even sound. This blueprint will help ensure the support you need for your journey.
